This document describes the privacy policy of Dandelion Hub for the management of your personal information. Dandelion Hub is bound by the legal requirements of the Australian Privacy Principles set out in the Commonwealth Privacy Act 1988. The Australian Privacy Principles govern the way in which we collect, use, disclose, store, secure and dispose of your Personal Information.

What is Personal Information and why do we collect it?
Personal Information is information or an opinion that identifies an individual. Examples of Personal Information we collect include: names, addresses, email addresses, phone numbers, health and medical information.
This Personal Information is obtained in many ways including interviews, consultations, forms, correspondence, telephone, email, register to our site, contact us, subscribe to our email list, or via our website www.dandelionhub.com.au, and from third parties, i.e. when other health practitioners provide personal information to Dandelion Hub via referrals, correspondence and medical reports. We don’t guarantee website links or policy of authorised third parties.
We collect your Personal Information for the primary purpose of providing our services to you, providing information to our clients, and marketing of our services. We may also use your Personal Information for secondary purposes closely related to the primary purpose, in circumstances where you would reasonably expect such use or disclosure. You may unsubscribe from our mailing/marketing lists at any time by contacting us in writing via email.
When we collect Personal Information we will, where appropriate and where possible, explain to you why we are collecting the information and how we plan to use it.
Third Parties
Where reasonable and practicable to do so, we will collect your Personal Information only from you. However, in some circumstances we may be provided with information by third parties, such as a referrer. In such a case we will take reasonable steps to ensure that you are made aware of the information provided to us by the third party and/or obtain your consent to collect such information.
Storage and Security of Personal Information
Your Personal Information is stored in a manner that reasonably protects it from misuse and loss and from unauthorized access, modification or disclosure. i.e., in password protected electronic document management systems, and on cloud based systems and servers, which is accessible only to authorised employees.
When your Personal Information is no longer needed for the purpose for which it was obtained, we will take reasonable steps to destroy or permanently de-identify your Personal Information. However, most of the Personal Information is or will be stored in client files which will be kept by us for a minimum of 7 years for adults, or longer for children.
Disclosure of Personal Information
Your personal information will remain confidential except when:
it is subpoenaed by a court, or disclosure is otherwise required or authorised by law; or
failure to disclose the information would in the reasonable belief of Dandelion Hub place a client or another person at serious risk to life, health or safety; or
your prior approval has been obtained to:
a) provide a written report to another agency or professional, e.g., a GP or a lawyer; or
b) discuss the material with another person, e.g. a parent, employer, health provider, or third party funder; or
c) disclose the information in another way; or
d) disclose to another professional or agency (e.g. your GP) and disclosure of your personal information to that third party is for a purpose which is directly related to the primary purpose for which your personal information was collected.
Dandelion Hub uses cloud services whose servers may be located in Australia or overseas. Your personal information may be stored on overseas servers, but is otherwise not disclosed to overseas recipients, unless you consent or such disclosure is otherwise required by law. Your personal information will not be used, sold, rented or disclosed for any other purpose. In the event that unauthorised access, disclosure or loss of your personal information occurs, Dandelion Hub will activate their data breach plan and use all reasonable endeavours to minimise any risk of consequential serious harm.
Requests for Access to your Personal Information
You may access the Personal Information we hold about you and to update and/or correct it, subject to certain exceptions. If you wish to access your Personal Information, please contact us in writing. Dandelion Hub may discuss the contents with you and/or give you a copy, subject to the exceptions in the Commonwealth Privacy Act 1988. If satisfied that personal information is inaccurate, out of date or incomplete, reasonable steps will be taken in the circumstances to ensure that this information is corrected. All requests by clients for access to or correction of personal information held about them should be lodged with Dr Ruth Tesselaar. These requests will be responded to in writing within 30 days, and an appointment will be made if necessary for clarification purposes.
Dandelion Hub will not charge any fee for your access request, but may charge an administrative fee for providing a copy of your Personal Information. In order to protect your Personal Information we may require identification from you before releasing the requested information.
Maintaining the Quality of your Personal Information
It is an important to us that your Personal Information is up to date. We will take reasonable steps to make sure that your Personal Information is accurate, complete and up-to-date. If you find that the information we have is not up to date or is inaccurate, please advise us as soon as practicable so we can update our records and ensure we can continue to provide quality services to you.
